If you are under 18 years of age and within the U.S., please have a parent or guardian complete the form for U.S. students.

If you are 18 or over, please sign the form for U.S. adults. If you are outside of the U.S. and under 18, please have a parent or guardian complete the form for non-US students.

If you are outside of the U.S. and 18 or over, please sign the form for non-US adults.

All participants need to have the confidential medical form completed by a physician.

All participants must bring these documents signed to the Institute in order to participate. No exceptions.
